Difficulty in applying form

My grand parents suffering form eye diseas but private hospital require 27 thousand for one eye enjection but my father don't have such investment. And I visit gov hospital they also charge high then I apply for this mukhyamantri yogna card and i visit to seva kendar but they told me there is no any yogna car is made.
