How to apply for the Pink Card free bus travel scheme in Kolkata?

Hello, I am a resident of Kolkata and want to register for the West Bengal Free Bus Travel Scheme for Women to get the Pink Card. Could someone please guide me on how and where to apply online or offline, and what exact documents are required? Thank you.
