I, SOBITA SINGHA, W/O, M. SURAJ KUMAR SINGHA resident of 114, Sonai Road, Dhonahori pt.3, P.O: Kajidhar, District: Cachar, State: Assam, Pin: 788115 humbly request financial assistance for my medical treatment under the Assam Arogya Nidhi scheme.
I was recently diagnosed with kidney stones in both kidneys and underwent surgery at INSTITUTE OF UROLOGY & KIDNEY DISEASES (IUKD) Hospital, Nazirakhat, Sonapur, Kamrup Assam - 782402. The treatment and surgery cost me a significant amount, and due to my family’s limited financial condition (annual income below ₹5 lakh), it is very difficult for us to bear these expenses.
I kindly request your office to consider my case and provide financial assistance under the Assam Arogya Nidhi. I am attaching the required documents including income certificate, identity proof, hospital bills, discharge summary, and bank details for your kind perusal.
