Dear Sir/Madam,
I respectfully submit that I had applied under the Namo Shri Yojana by submitting my YIWI (Your Identity With Income) based application form. However, despite repeated follow-ups over the last 2.5 years, I have neither received any response nor any benefits under this scheme.
I am therefore compelled to bring to your notice the difficulties and harassment I have faced at Diwalipura Urban Primary Health Centre (UPHC), Vadodara.

Complaint Details:
Even after 2.5 years, I have not received any benefit under the scheme.
At Diwalipura UPHC, staff including the Aasha worker have not provided proper support or guidance.
I have been repeatedly asked to visit different offices without proper assistance.
My wife is pregnant, and despite this, we were told to visit during 2:00 PM to 5:00 PM, which is extremely difficult in high temperatures (45°C–50°C).
We were also told to go to “Ayushman” for processing, which caused further confusion and inconvenience.
Such treatment is causing unnecessary stress and hardship, especially for a pregnant woman.
Government schemes like Namo Shri Yojana are meant to support pregnant women and ensure proper care and nutrition. However, due to mismanagement and lack of cooperation at the local level, we are being harassed instead of being helped.
