ಯೋಜನೆಯ ಹೆಸರು ಕರ್ನಾಟಕ ಸಮಗ್ರ ಮಾತೃಆರೋಗ್ಯ ಪಾಲನೆ ಯೋಜನೆ.
ಉಪ ಯೋಜನೆಗಳು
ಪ್ರಯೋಜನಗಳು ಕರ್ನಾಟಕ ರಾಜ್ಯದ ಗರ್ಭವತಿ ಹಾಗೂ ಹಾಲುಣಿಸುವ ಮಹಿಳೆಯರು.
ನೋಡಲ್ ಡಿಪಾರ್ಟ್ಮೆಂಟ್ ಕರ್ನಾಟಕ ರಾಜ್ಯದ ಆರೋಗ್ಯ ಮತ್ತು ಕುಟುಂಬ ಕಲ್ಯಾಣ ಇಲಾಖೆ.
ಅರ್ಜಿಯ ಮೋಡ್ ಯಾವುದೇ ರೀತಿಯ ಅರ್ಜಿ ಸಲ್ಲಿಸುವುದು ಅವಶ್ಯಕತೆ ಬರುವುದಿಲ್ಲ.

ಯೋಜನೆಯ ಪರಿಚಯ

  • ಕರ್ನಾಟಕ ಸಮಗ್ರ ಮಾತೃ ಆರೋಗ್ಯ ಪಾಲನೆ ಯೋಜನೆಯು ಗರ್ಭಿಣಿಯರ ಕಲ್ಯಾಣಕ್ಕಾಗಿ ಕರ್ನಾಟಕ ಸರ್ಕಾರದ ಪ್ರಮುಖ ಕಲ್ಯಾಣ ಯೋಜನೆಯಾಗಿದೆ.
  • ಈ ಯೋಜನೆಯನ್ನು ಪ್ರಾರಂಭಿಸುವುದರ ಮುಖ್ಯ ಉದ್ದೇಶವೇನೆಂದರೆ ರಾಜ್ಯದ ಗರ್ಭಿಣಿಯರಿಗೆ ಈ ಕೆಳಗಿನ ಸೌಲಭ್ಯಗಳನ್ನು ಒದಗಿಸುವುದು :-
    • ಸರ್ಕಾರಿ ಮತ್ತು ಖಾಸಗಿ ಆಸ್ಪತ್ರೆಗಳಲ್ಲಿ ಉಚಿತ ಹೆರಿಗೆ ಸೇವೆಗಳು.
    • ಹೆರಿಗೆ ಪೂರ್ವ ಮತ್ತು ನಂತರದ ವೆಚ್ಚಗಳನ್ನು ನೋಡಿಕೊಳ್ಳಲು ಹಣಕಾಸಿನ ನೆರವು.
    • ಮಡಿಲು ಕಿಟ್ ಅನ್ನು ಒದಗಿಸುವುದು 19 ದೈನಂದಿನ ಬಳಕೆಯ ವಸ್ತುಗಳನ್ನು ಒಳಗೊಂಡಿದೆ.
  • ಆರೋಗ್ಯ ಮತ್ತು ಕುಟುಂಬ ಕಲ್ಯಾಣ ಇಲಾಖೆಯು ಈ ಯೋಜನೆಯ ನೋಡಲ್ ಇಲಾಖೆಯಾಗಿದೆ.
  • ಕರ್ನಾಟಕ ಸಮಗ್ರ ಮಾತೃ ಆರೋಗ್ಯ ಪಾಲನೆ ಯೋಜನೆಯು 4 ಪ್ರಮುಖ ಉಪ ಯೋಜನೆಗಳನ್ನು ಹೊಂದಿದೆ :-
  • ಪ್ರತಿಯೊಂದು ಉಪ ಯೋಜನೆಯು ತನ್ನದೇ ಆದ ಪ್ರಯೋಜನಗಳನ್ನು ಹೊಂದಿದೆ ಮತ್ತು ಕರ್ನಾಟಕ ರಾಜ್ಯದ ಗರ್ಭಿಣಿ ಮತ್ತು ಹಾಲುಣಿಸುವ ತಾಯಿಗೆ ಬಹಳ ಪ್ರಯೋಜನಕಾರಿಯಾಗಿದೆ.
  • ತಾಯಿ ಭಾಗ್ಯ ಯೋಜನೆಯಡಿ ಈ ಕೆಳಗಿನ ಸೇವೆಗಳು ಲಭ್ಯವಿರುತ್ತವೆ :-
    • ಖಾಸಗಿ ಅಥವಾ ಸರ್ಕಾರಿ ಆಸ್ಪತ್ರೆಯಲ್ಲಿ ಉಚಿತ ಹೆರಿಗೆ.
    • ಗರ್ಭಧಾರಣೆಯ ಪೂರ್ವ ಮತ್ತು ನಂತರದ ಅವಧಿಯಲ್ಲಿ ನಗದುರಹಿತ ಚಿಕಿತ್ಸೆ.
    • ಉಚಿತ ಸಾರಿಗೆ ಸೌಲಭ್ಯ.
    • ವೆಚ್ಚದ ತಪಾಸಣೆ ಮತ್ತು ಔಷಧಿಗಳ ಉಚಿತ.
  • ತಾಯಿ ಭಾಗ್ಯ ಪ್ಲಸ್ ಯೋಜನೆಯು ರೂ.1000/- ಗಳ ಆರ್ಥಿಕ ನೆರವು ನೀಡುತ್ತದೆ. ಎಂಪನೆಲ್ ಮಾಡದ ಖಾಸಗಿ ಆಸ್ಪತ್ರೆಗಳಲ್ಲಿ ತನ್ನ ಮಗುವನ್ನು ಹೆರಿಗೆ ಮಾಡಲು ಆಯ್ಕೆ ಮಾಡುವ ಗರ್ಭಿಣಿಯರಿಗೆ.
  • ಪ್ರಸೂತಿ ಅರೈಕೆ ಯೋಜನೆಯು ಒದಗಿಸುತ್ತದೆ :-
    • ಆರ್ಥಿಕ ನೆರವು ರೂ. 3,000/- SC ಮತ್ತು ST ಹಾಲುಣಿಸುವ ತಾಯಿಗೆ.
    • ಆರ್ಥಿಕ ನೆರವು ರೂ. 2,000/- ಬಿಪಿಎಲ್ ವರ್ಗದ ಹಾಲುಣಿಸುವ ತಾಯಿಗೆ.
  • ಮಡಿಲು ಕಿಟ್ ಯೋಜನೆಯಡಿ, 19 ದೈನಂದಿನ ಬಳಕೆಯ ವಸ್ತುಗಳ ಕಿಟ್ ಸುಮಾರು ರೂ. ಚಿಲ್ ಅನ್ನು ವಿತರಿಸಿದ ನಂತರ ಮಹಿಳೆಯರಿಗೆ 1500/- ನೀಡಲಾಗುತ್ತದೆ.
  • ಯು ಗರ್ಭಿಣಿಯರು, ಹಾಲುಣಿಸುವ ತಾಯಿ ಮತ್ತು ಅವರ ನವಜಾತ ಶಿಶುವಿಗೆ ಸುರಕ್ಷಿತ ವಾತಾವರಣವನ್ನು ನಿರ್ಮಿಸುತ್ತಿದೆ.
  • ಇದು ರಾಜ್ಯದಲ್ಲಿ ಶಿಶು ಮರಣ ಪ್ರಮಾಣವನ್ನು ಕಡಿಮೆ ಮಾಡಲು ಸಹಾಯ ಮಾಡುತ್ತದೆ.
  • ಪರಿಶಿಷ್ಟ ಜಾತಿ, ಪರಿಶಿಷ್ಟ ಪಂಗಡ ಮತ್ತು ಬಿಪಿಎಲ್ ವರ್ಗಕ್ಕೆ ಸೇರಿದ ಮಹಿಳೆಯರು ಸಮಗ್ರ ಮಾತೃ ಆರೋಗ್ಯ ಪಾಲನೆ ಯೋಜನೆಯಡಿ ಅರ್ಜಿ ಸಲ್ಲಿಸಲು ಅರ್ಹರು.
  • ಅರ್ಹ ಫಲಾನುಭವಿಗಳು ಕರ್ನಾಟಕ ಸಮಗ್ರ ಮಾತೃ ಆರೋಗ್ಯ ಪಾಲನೆ ಯೋಜನೆಯ ಉಪ ಯೋಜನೆಗಳ ಪ್ರಯೋಜನವನ್ನು ಪಡೆಯಲು ಆಶಾ ಕಾರ್ಯಕರ್ತೆ ಅಥವಾ ANM ಅವರನ್ನು ಸಂಪರ್ಕಿಸಬಹುದು.

ಪ್ರಯೋಜನಗಳು

  • ಖಾಸಗಿ ಹಾಗೂ ಸರ್ಕಾರಿ ಆಸ್ಪತ್ರೆಗಳಲ್ಲಿ ಉಚಿತ ಹೇಗೆ.
  • ಉಚಿತ ಟ್ರಾನ್ಸ್ಪೋರ್ಟೇಶನ್ ಸೇವೆ.
  • ಹೆರಿಗೆ ಪೂರ್ವ ಹಾಗೂ ಹೆರಿಗೆಯ ನಂತರ ಮಗದುರಹಿತ ಸೇವೆ.
  • ಉಚಿತ ಚೆಕಪ್ ಹಾಗೂ ಔಷಧಿಗಳು.
  • ಮಡಿಲು ಕಿಟ್ 19 ವಸ್ತುಗಳನ್ನು ಒಳಗೊಂಡಿದೆ ಸುಮಾರು ರೂ. 1,500/-.
  • ಆರ್ಥಿಕ ನೆರವು ರೂ. 3,000/- SC ಮತ್ತು ST ಹಾಲುಣಿಸುವ ತಾಯಿಗೆ.
  • ಆರ್ಥಿಕ ನೆರವು ರೂ. 2,000/- ಬಿಪಿಎಲ್ ವರ್ಗದ ಹಾಲುಣಿಸುವ ತಾಯಿಗೆ.
  • ಆರ್ಥಿಕ ನೆರವು ರೂ. 1,000/- ಎಂಪನೆಲ್ ಮಾಡದ ಖಾಸಗಿ ಆಸ್ಪತ್ರೆಯಲ್ಲಿ ಮಗುವನ್ನು ಹೆರಿಗೆ ಮಾಡಲು.

ಅರ್ಹತೆ

  • ಕರ್ನಾಟಕ ರಾಜ್ಯದ ನಿವಾಸೆ.
  • ಈ ಕೆಳಗಿನ ಮಹಿಳೆಯರು ಅರ್ಹರು :-
    • ಂಪನೆಲ್ ಮಾಡದ ಖಾಸಗಿ ಆಸ್ಪತ್ರೆಯಲ್ಲಿ ತನ್ನ ಮಗುವನ್ನು ಹೆರಿಗೆ ಮಾಡುವ ಮಹಿಳೆಯರು.
    • ಬಿಪಿಎಲ್ ವರ್ಗಕ್ಕೆ ಸೇರಿದ ಮಹಿಳೆಯರು.
    • ಪರಿಶಿಷ್ಟ ಜಾತಿ/ಪಂಗಡದ ವರ್ಗಕ್ಕೆ ಸೇರಿದ ಮಹಿಳೆಯರು.

ಅಗತ್ಯವಾದ ದಾಖಲೆಗಳು

  • ಕರ್ನಾಟಕದ ನಿವಾಸ ಪುರಾವೆ.
  • ಬಿಪಿಎಲ್ ಕಾರ್ಡ್ ಅಥವಾ ಪಡಿತರ ಚೀಟಿ.
  • ANC ನೋಂದಣಿ ಸಂಖ್ಯೆ.
  • ಮೊಬೈಲ್ ನಂಬರ.
  • ಬ್ಯಾಂಕ್ ವಿವರಗಳು.
  • ಜಾತಿ ಪ್ರಮಾಣ ಪತ್ರ.
  • ಆಧಾರ್ ಕಾರ್ಡ್.

ಅರ್ಜಿ ಸಲ್ಲಿಸುವ ವಿಧಾನ

  • ಕರ್ನಾಟಕ ಸಮಗ್ರ ಮಾತ್ರ ಆರೋಗ್ಯ ಪಾಲನೆ ಯೋಜನೆ ಅಡಿ ಯಾವುದೇ ಅರ್ಜಿಯೂ ಸಲ್ಲಿಸುವ ಅಗತ್ಯವಿಲ್ಲ.
  • ಫಲಾನುಭವಿಗಳು ತಮ್ಮ ಪ್ರದೇಶದ ಆಶಾ ಕಾರ್ಯಕರ್ತೆ ಅಥವಾ ಕಿರಿಯ ಮಹಿಳಾ ಆರೋಗ್ಯ ಅಧಿಕಾರಿಯನ್ನು ಸಂಪರ್ಕಿಸಬಹುದು.
  • ಪ್ರತಿ ಯೋಜನೆಯಲ್ಲಿ ಫಲಾನುಭವಿಗಳನ್ನು ಪ್ರತ್ಯೇಕವಾಗಿ ನೋಂದಾಯಿಸಿಕೊಳ್ಳಬಹುದು.
  • ಆಶಾ ಕಾರ್ಯಕರ್ತೆ ಅಥವಾ ಕಿರಿಯ ಕುಟುಂಬ ಆರೋಗ್ಯ ಅಧಿಕಾರಿಯು ಮಹಿಳೆಯರನ್ನು ನೋಂದಾಯಿಸುತ್ತಾರೆ ಮತ್ತು ಹೆಚ್ಚಿನ ಅನುಮೋದನೆಗಾಗಿ ಅರ್ಜಿಯನ್ನು ರವಾನಿಸುತ್ತಾರೆ.
