Eligibility Requirements

• The worker must be employed in an establishment registered under the Factories Act, 1948.
• The applicant must be a permanent resident of Uttar Pradesh.
• The worker's monthly income (basic salary + DA) must not be more than Rs 24,000.
• The application should be submitted within one year of the worker's death.
• Financial support will be provided to the spouse or dependent children (son/unmarried daughter), and if the worker was unmarried, then to the parents.

• Residential Proof document (Adhar Card/Domicile)
• Phone number/Email ID
• Copy of the beneficiary's bank passbook showing account details and IFSC code.
• Document proving relationship with the deceased (such as a ration card, family register, or any valid government record).
• Official death certificate issued by the Registrar of Births and Deaths.
• Scanned and verified copy of the submitted online application form.

Steps to Apply

• First, visit the official website of the "Labour Welfare Board Department of Uttar Pradesh.
• Scroll and click on “Labour Application” (skip this step if you are already registered).
• If you are a new user, select “Register New User”, complete the registration form, and submit it. Step 4: You will receive your User ID and password on your registered mobile number.
• Log in using your User ID and password.
• Choose the “Dattopant Thengadi Mrutak Shramik Aarthik Sahayata Yojana” from the list of schemes, fill out the application form, upload your photograph, and submit it.
• After submission, print a copy of your application form.
• Get the printed application verified by the concerned factory or establishment.
• Log in again to your account, go to “Scheme Application Details”, upload the scanned copy of the verified application along with all required documents, and save it.
• Once your application is successfully verified, the financial assistance amount will be directly credited to your bank account. You will receive an SMS confirmation on your registered mobile number.
• You can track the status of your application at any time by clicking on “Application Status” on the portal.
